Since the middle seat costs the same but sucks in every other way, that structure is what most frequent fliers consider "fair."
No one should need to use the arm rest, that way people can fit next to each other without being in direct contact. The airline policies say that it's OK if all three people need to take up 100% of the arm rests on both sides of them... which means that their arms are all crammed into the same space as each other. That's not "fitting" at all. How is this not obvious?
